Résumé

Les lymphomes primitifs du testicule présentent des particularités histologiques, évolutives et pronostiques. Il s’agit le plus souvent de lymphome diffus B à grandes cellules. Ils sont associés à une grande fréquence de récidives extraganglionnaires. Le lymphome du testicule atteint principalement l’adulte âgé mais de rares cas sont décrits chez l’enfant. Son traitement comporte le plus souvent un geste chirurgical diagnostique. La stratégie thérapeutique actuelle associe les protocoles modernes de chimiothérapie des lymphomes incluant le rituximab. Toutefois plusieurs questions se posent concernant la place éventuelle de l’irradiation prophylactique du testicule restant, des aires ganglionnaires de drainage et la place de la prophylaxie cérébrale ainsi que de ses modalités.
