Cardiovascular Risk Prediction by Measurement of Arterial Elastic Properties and Wall Thickness

Abstract

Arterial elastic properties (stiffness, wave reflections, and central pressures) and carotid wall thickness are important determinants of global cardiovascular (CV) performance. There is mounting evidence that these arterial characteristics are associated with the presence and extent of CV disease. Furthermore, they are important and independent predictors of CV risk in several populations, and may even be used to monitor the clinical benefit of treatment strategies. Recent guidelines underscore the value of aortic pulse wave velocity (PWV) and carotid intima-media thickness (IMT) as tools for stratification of risk in hypertensive patients. Currently, there are many available modalities for the evaluation of arterial elastic properties, and several imaging protocols are devoted to measuring the thickness of the carotids. Recently, attempts to define the normal values for arterial indexes and to standardize the available techniques have been forthcoming. It is anticipated that these efforts will facilitate the implementation of the concept of “vascular age” in everyday clinical practice.
