Abstract
There are real concerns regarding continence outcome after lateral internal anal sphincterotomy in selected patients. In patients with persistent symptomatic fissure after definitive surgery, the risks to continence are particularly real. This chapter outlines the place for various sphincter-sparing surgical alternatives when conventional surgery fails.
