Abstract
Chinese MPCNL is not a simple type of PCNL using small tracts. We have standardized the use of the equipment and simplified the surgical techniques. Only one type of guidewire, a set of fascial dilator and peel-away sheath, a type of endoscope, forceps, and intracorporeal lithotripter are needed. The preferred puncture site is through a posterior middle calyx. We use a specially designed pulsatile high-pressurized endoscopic perfusion pump (MMC Yiyong, Guangzhou, China) via a specially designed Lixun nephroscope (Richard Wolf, Knittlingen, Germany) to produce forceful pulse flow of irrigant to flush out the stone fragment, which avoids the tedious process of picking up every fragment by forceps and shortened the operative time. With these modifications, the indication of Chinese MPCNL can be widened to all the upper urinary tract stones that need PCNL intervention. The safety and efficiency of Chinese MPCNL has been confirmed by our and others’ clinic data. In this section, we report the indication, equipment, and surgical procedures of Chinese MPCNL and report our experience of Chinese MPCNL in managing a case of staghorn stone complicated with severe spinal deformity.
