Sensory Neuromodulators in Disorders with Delayed Gastric Emptying

Abstract

There is considerable overlap in clinical presentation between functional dyspeptic and vomiting syndromes associated with inherent delay in gastric emptying and neuromuscular dysfunction of gastric emptying mechanisms resulting in true gastroparesis. Symptomatic foregut disorders with delayed gastric emptying may respond to treatment with sensory neuromodulators. Tricyclic antidepressants are the agents used most often, and as many as three quarters of patients may demonstrate moderate symptom improvement. Systematic, large clinical trials using these agents do not exist, however, and much of the experience has been extracted from the use of sensory neuromodulators in overlapping functional syndromes. In this chapter, the role of sensory modulators in managing foregut symptoms is discussed, side effects are characterized, and guidelines for use are detailed.
