Abstract
Rest tremor is generally regarded as the characteristic tremor which is associated with Parkinson’s disease (PD). However, some patients with PD also experience a postural tremor which may be unilateral or bilateral and may be more severe and disabling than their rest tremor. If other signs of PD are not yet present, a misdiagnosis of essential tremor (ET) is often made. The postural tremor of ET occurs immediately after the upper limbs are raised to a horizontal position, while the postural tremor of PD occurs after a latency of about 5 s and has the same frequency as the patient’s rest tremor.

The patient shows continuous rest tremor in left arm and leg. With arms extended, the right arm tremor improves and then reappears while he recites the months of the year. It later appears spontaneously just prior to being asked to recite months of year backward a second time.
