The Role of Obesity in Insulin Resistance

Abstract

Obesity is a rapidly growing health problem in the United States. In 2000, about 20% of U.S. adults self-reported a body mass index (BMI) exceeding 30 kg/m2, a 61% increase in obesity since 1991 (1). This underestimates the real prevalence when body mass index is determined from direct measurements rather than self-report. More than half (56%) reported being at least overweight (BMI ≥25 kg/m2), up from 45% in 1991. Lifetime risk for overweight during the 1990s was approximately 50% for overweight and 25% for obesity in a representative U.S. sample (2). Obesity confers substantial excess risk for morbidity and mortality from a variety of associated conditions, especially type 2 diabetes and atherosclerotic cardiovascular disease (CVD) (35). Although overall body fatness, as measured by BMI, is clearly associated with mortality and complications of obesity (6), fat distribution, especially central fatness as measured by a large waist circumference, may be a more powerful correlate of risk associated with obesity, especially for type 2 diabetes and CVD (79).
