Abstract
Step 3 requires selecting the correct level for history and physical examination. Although it might seem logical to divide these two components into two separate steps, they are both determined essentially the same way. The level of service for both history and physical are determined by counting the number of items evaluated and documented from a list determined by CMS. It is essentially the number of items that determines the correct level of service.
