Abstract
A 48-year-old man was brought to the ER with a 1-week history of confusion with marked lethargy for 24 hours. He had a history of chronic alcohol use and poor nutrition. On examination, he was poorly responsive to external stimuli with intermittent eye opening to commands. Cranial nerve examination revealed sluggish pupillary light responses, vertical nystagmus on upgaze, and abduction paresis with horizontal end-gaze nystagmus on lateral gaze bilaterally. Further neurological examination revealed symmetrical localization to noxious stimuli in all limbs with severe truncal ataxia in a sitting position.
