Abstract

In the last few years, the adult onset of globoid cell leukodystrophy or Krabbe disease has been recognized. A few authors have described an association of progressive spastic paraparesis and selective involvement of the corticospinal tracts in adult patients with deficiency of galactocerebrosidase, the Krabbe disease-related enzyme. This phenotypic expression of the disease should be considered in the differential diagnosis of spastic paraparesis. Magnetic resonance imaging may offer the clues for the recognition of this disorder. Proton MR spectroscopy may be useful in quantifying the damage to the corticospinal tract.
