Growth Hormone and Growth Hormone Secretagogues in Adults

Abstract

The association of growth hormone (GH) with the promotion of linear growth in childhood has focused attention away from its role in adults, but GH continues to play an important metabolic role in adult life, as a partitioning hormone regulating body composition and function. Patients who aquire GH deficiency (GHD) in adult life lack the obvious failure of linear growth that brings GHD children to medical attention, but they have an increase in cardiovascular and cerebrovascular mortality and well-docu­mented, clinically significant abnormalities in hormone profiles, body composition, and physical and mental functions that are largely reversed by treatment, with some studies now reporting durable benefits over as long as 10 yr of treatment. This has led to a consensus that in most GHD patients, GH replacement should be continued indefinitely even after final adult stature is attained. Although adults are often more sensitive to the side effects of GH and there are still no data directly demonstrating improved functional performance, GH treatment of GHD adults has been approved by drug-regulatory agen­cies in the United States, Europe, and other regions.
