Abstract

Neuro-orthopedics is the field of orthopedic surgery that treats limb deformities resulting from neurologic disease or injury. In this chapter we present the neuro-orthopedic perspective on managing patients with upper motor neuron (UMN) dysfunction. UMN dysfunction commonly follows injuries to the brain from stroke, traumatic brain injury, anoxia, infections of the brain, and perinatal trauma (i.e., cerebral palsy [CP]or static encephalopathy) (1–5). The approach to management of limb dysfunction follows the same principles regardless of etiology. A number of common patterns of movement dysfunction are seen in patients with UMN dysfunction (see Table 1). These patterns typically develop against a background of weakness, impaired voluntary and postural motor control, spasticity, and contracture that comes with the territory of UMN dysfunction.
