Abstract
The delivery of health care has undergone dramatic changes in the last decade. The invention of managed care has had a profound impact on the practice of urology. Managed care, as well as advancement in technology, has decreased the need for urologists’ services, resulting in a price-competitive environment. Our reimbursement is based on contracts; therefore, the business aspect of our practice is becoming more important. We are asked to deliver high-quality care at a lower cost. Capitation places even higher demands on those who accept this type of reimbursement. The capitated payment system works to the payer’s advantage, guaranteeing a certain rate of profits while shifting the risk to providers. Having the precise knowledge of the cost of services provided—as well as the probability that those services will be needed—is the key to success. We are required to keep meticulous medical records and to comply with coding and reimbursement rules that often vary from payer to payer. New confidentiality regulations regarding medical records will require that we keep accurate logs about access to documents. Because of the time and effort needed to keep up with regulations and requirements, the practice of medicine in today’ s society is demanding for physicians and their staff. Managed care and government regulations have significantly increased the amount of paperwork we deal with, thus increasing the cost of running our practices.
