Abstract

The MitraClip device was conceived of as a percutaneous method to mimic the surgical edge-to-edge or double-orifice mitral repair. A surgical double-orifice repair was initially utilized in patients with degenerative mitral regurgitation (MR), involving prolapse of the middle scallops of the line of mitral coaptation. The EVEREST II pivotal trial compared MitraClip with surgical mitral repair or replacement in a predominantly degenerative MR population and demonstrated that surgical repair is more effective at reducing MR than MitraClip, but that the MitraClip device achieved similar levels of clinical efficacy and left ventricular remodeling and is, as was expected, safer than surgical approaches. Clinical uptake of MitraClip in commercial international practice has been used mostly among high-risk patients with functional MR with similar efficacy and safety, and over 25,000 patients have been treated. The evidence base for use of MitraClip now includes the EVEREST trials and prospective registry data for over 5000 patients. This review summarizes this database.
