Non-endoscopic Management of Acute Mechanical Colonic Obstruction and Pseudo-obstruction

Abstract

Acute colonic obstruction is either due to acute colonic pseudo-obstruction (ACPO) or mechanical colonic obstruction (MCO). The clinical presentation is similar in both conditions. Radiologic imaging is the principal determinant for the diagnosis and differentiation of ACPO and MCO. The evaluation and management of ACPO include careful review and modification of predisposing factors, including metabolic derangements and medications that impair motility. Management options for ACPO include pharmacologic agents that promote colonic motility and endoscopic decompression. Acute MCO generally requires endoscopic or surgical intervention for definitive therapy. A multidisciplinary approach, with the involvement of gastroenterologists, surgeons, and interventional radiologists, will optimize patient outcomes.
