Abstract
Natural orifice translumenal endoscopic surgery is a new surgical paradigm combining many facets of advanced therapeutic endoscopy and laparoscopic surgery. Much of the groundbreaking work in NOTES surgery is centered on transoral/transgastric approaches. Transanal NOTES, however, has many potential advantages over transoral approaches in selected situations. Surgeons have long since utilized the transanal route to remove sigmoid colon specimens during laparoscopic left colon surgery. Transanal total mesorectal excision has several appealing benefits and is being investigated as the first major transanal NOTES procedure.
