Abstract

Compression, distortion or occlusion of the bladder outlet impedes flow of urine during voiding, leading to characteristic symptoms of hesitancy, poor stream, dribbling and incomplete emptying. Benign prostate enlargement (BPE) and urethral stricture are the most common causes in men. Secondary changes in the lower urinary tract can result, which affect the structure and physiology and may additionally lead to storage phase symptoms, though the latter may also arise incidentally. Diagnostic tests are needed to understand mechanisms and contributory factors. Most men are investigated with measurement of free flow rate tests and post-void residual. In men with BPE, invasive urodynamic testing enables confirmation of bladder outlet obstruction and exclusion of detrusor underactivity. In urethral stricture disease, endoscopic examination and imaging are used to obtain the diagnosis and severity of the problem.
