Abstract
Our greatest opportunity to enhance value in US healthcare is improving quality of care for older adults with serious illness. Patients with serious illness and their families value care which promotes function, comfort, and shared decision-making. Interdisciplinary palliative care and hospice teams are the experts, but quality standards need to extend to other providers and settings. Researchers, clinicians, and policy-makers are working to design quality measures matched to serious illness. Strategies for meaningful use in the future include identifying patients with serious illness in all settings, measuring what matters to patients, and making measures responsive to preferences.
