Abstract
Symptomatic spondylolisthesis has three main elements: symptoms due to neurologic compression, progressive spondylolisthesis, and low back pain arising from instability and loss of sagittal balance. This chapter focuses on surgical decompression for neurologic compression and fusion with and without instrumentation for instability. Local segmental balance and global sagittal balance is a key component of adequate treatment and better outcomes.
