Use of colon as an oesophageal replacement

  • Chapter
Book cover Surgery of the Upper Gastrointestinal Tract

Abstract

The use of long segments of colon for replacement or bypass of all or part of the thoracic oesophagus was introduced independently by Kelling1 and Vulliet2 in 1911. Subsequently, the interposed colon has emerged as a durable, well functioning substitute for the oesophagus. Initially it was used in patients with malignancy of the upper oesophagus. Subsequently, with refinements in technique and improved results it, rather than the stomach, has been used for an oesophageal substitute in advanced benign disease or early malignancy arising in the distal oesophagus, cardia and proximal stomach, particulary when it is expected that the replacement must function for a decade or more.
