Abstract
This chapter considers two aspects of standard setting: the content and nature of standards and the process of monitoring them. Both are problematic. Concern about standards of care for elderly patients in institutions rightly persist, shared by leaders of NHS professional groups: “ ... while there has been considerable progress, inhumanity and inadequate care have not been eradicated from every ward where old people are treated” (Royal College of Nursing, 1987). Arrangements for monitoring public and private provision also have many critics, partly because they have not rooted out poor standards. They have proved inadequate for the task.
