Abstract

Evaluation and treatment of the dual diagnosis patient during an acute drug-related episode or psychiatric decompensation can be among the most difficult tasks faced by the clinician. There are few rules because each patient must be evaluated individually. Intoxication, overdose, and withdrawal will have physical, psychiatric, and neurologic manifestations. These will vary from person to person and can be particularly confusing in someone who has a coexisting psychiatric illness, which many substance abusers apparently have. For example, Rounsaville et al. found that in a sample of 298 cocaine abusers seeking treatment, 55.7% met current and 73.5% met lifetime criteria for a psychiatric disorder other than a substance use disorder (Rounsaville, Anton, Carroll, Budde, Prusoff, and Gawin, 1991). This chapter will examine some of the general issues regarding these acute events as they relate to dual diagnosis patients and then look at problems relating to specific drugs of abuse.
