Abstract

The incidence of insulin-dependent diabetes mellitus (IDDM) in Caucasians shows remarkable differences, with rates much lower in Central and Southern Europe than in Northern Europe. On the basis of these data, a North-South gradient has been proposed. However, this idea has recently been challenged by the incidence data from Sardinia, an island located in the central part of the Mediterranean Sea. The Sardinian population has an extremely high incidence of IDDM1,2 (30.2 cases per 100,000per year; the second highest rate recorded in Europe). It may be considered, due to the high degree of genetic homogeneity, a convenient place for genetic studies. As far as HLA genes are concerned, several studies have shown some peculiarities of our population.3 For example, the extended haplotypes A30, B18, DR3, DRw52, DQw23 and A2, Cw7, B17, 3F31, DR2, DQw1,4 rare in other Caucasians, are very frequent on our island.
