Chuck Leonard’s Operations Group had a very broad mission. Not only was it charged with finding out how USAir Flight 427 was piloted on its doomed flight, it had to probe, in the minutest detail, the background of the crew and the airline’s procedures and practices. Leonard wanted to know, for example, how USAir recruited and trained its pilots, how their performance was scrutinized, and how they were trained to handle emergencies. Was there anything about the safety culture at USAir that might explain this and any other accidents? Was the aircraft fueled and loaded properly? Had the aircraft, in its maintenance schedule, shown any recurring or unrectified problems? Were the pilots fatigued?
