Abstract
When I decided to write the present book, I promised my editor that I would have it to him within a year. I underestimated my commitment. Instead of one year, it has taken me three, mainly because I was unable to reconcile the many contradictions and the new ideas that now characterize this literature. A mere listing of the references I had cited in my earlier drafts ran to over 100 pages!
