Abstract

Assessment of personality disorders, AXIS II conditions in the 5-axes diagnostic system of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) (American Psychiatric Association, 1994) is an often neglected but important area of concern. Comorbidity of Axis I and Axis II disorders is extremely common (Dolan-Sewell, Krueger, & Shea, 2001). The presence of a personality disorder can greatly complicate treatment of co-occurring Axis I disorders, simply by adding to the complexity of the presenting problem, but also through interfering with or otherwise disrupting the therapeutic alliance. In addition, when Axis II pathology is unrecognized or mistaken as an AXIS I condition, the therapist can be misled in attempts to develop an appropriate treatment plan. Until recently, prognosis for treatment of personality disorders has been quite poor. However, with publication of empirical studies showing effectiveness of some manualized, cognitive behavioral approaches, such as Linehan’s (1993) Dialectical Behavior Therapy for the treatment of Borderline Personality Disorder (BPD), perceptions of what is clinically feasible in treating Axis II conditions is rapidly changing.
