A Polyherbal Formulation for a Wide Spectrum of Reproductive Tract and Sexually Transmitted Infections

Sexually transmitted infections (STI) are on an increase worldwide. According to WHO 1, 333 million new cases of STIs occur globally every year excluding HIV, and HIV is assuming pandemic proportions in many countries. Besides this, reproductive tract infections (RTIs) caused by aerobic, anaerobic bacteria, protozoa, yeast (Candida) etc, manifested in the syndrome of abnormal vaginal discharge (AVD) are widespread. Its prevalence is around 30% in women visiting public hospitals in cities in India2 and is over 50% in rural areas3. One billion events of bacterial vaginosis are believed to occur globally in a year4. Although antibiotics and oral ± local vaginal use therapies are available for most of the pathogens, their rational use requires diagnosis of the causative organism, but facilities for their diagnosis are not available universally, specially in economically developing countries. The objective of our work was to develop a vaginal formulation, whose use be totally under the control of the woman, and which could protect her as much as possible from sexually transmitted infections. In addition, the formulation as a single locally applied “drug” was aimed to act across the spectrum of micro-organisms responsible for the syndrome of AVD. This charter demanded not one but a combination of ingredients as there is no single compound known which acts on all micro-organisms responsible for AVD. Furthermore, as a frequently used preparation, it has to be safe and free from causing inflammation or mucosal damage, which the presently marketed vaginal suppositories, creams, sponges do. These preparations are based on powerful surfactants such as Nonoxynol-9 (N-9), benzalkonium chloride etc.
