A great variety of diseases or conditions produce fever (Table 57.1). Infection is one of the most common. Exogenous pyrogens (endotoxin, bacteria, immune complexes, or viruses) stimulate neutrophils and other phagocytic cells to release endogenous pyrogens, which act on the hypothalamus to produce shivering and vasoconstriction, which in turn produces fever.1
