Vitrectomy Fluid-Gas Exchange in the Treatment of Macular Hole

Abstract

In the past retinal detachment (R.D.) by macular hole (M.H.) always presented considerable problems both from the surgical point of view and functional recovery. Many surgeons studied the results of the use of several techniques in order to solve these problems, but much too often these attempts were unsuccessful, as they presented a great number of intraoperative and postoperative complications, which were quite difficult to manage (1–5,7–22). Later on with the advent of vitrectomy, the pathogenesis of retinal detachment seemed to be connected with vitreous traction; when this concept was accepted it was quite easy to think of managing R.D. by M.H. with vitrectomy and internal tamponade (Gonvers, Machemer, Tittarelli-Benedetti).
