Clinically apparent icterus in a neonate generally occurs when the serum bilirubin exceeds 5 mg/dl. This level may be somewhat lower for an older child. The bilirubin can be either unconjugated (indirect) or conjugated (direct), with the differential diagnosis for each being very different.
