Abstract
In July 1987 a multicenter prospective, randomized study was begun in cooperation with other German stone centers, in which the modified Dornier HM3 generator system has been installed. Applying a complex classification of staghorn stones and collecting system, the following criteria are evaluated after at least three-month follow-up:

Rate of recurrent or persisting infection

Functional behavior of the affected kidney

Rate of auxiliary measures

Perioperative morbidity and in-hospital stay
Complete follow-up data are available on the first 33 patients (20 ESWL monotherapy, 13 combined PNL/ESWL). Hospital stay was shorter with ESWL monotherapy (24.3 days v 32.8 days). There were six failures (30%) in the ESWL monotherapy group and no failures in the combined PNL/ ESWL group. The stone-free rate for ESWL monotherapy (excluding failures) was 42.8% while the stone-free rate for combined PNL/ESWL was 52.8%. ESWL monotherapy and combined PNL/ ESWL appear in these preliminary results to provide comparable results in the management of staghorn calculi. ESWL monotherapy has the advantage of being less invasive.
