Incidence of Nephropathy in Insulin-Dependent Diabetes as Related to Mortality

Abstract
Epidemiologic studies of patients with insulin-dependent diabetes mellitus (IDDM) have consistently shown that IDDM patients are characterized by a mortality rate that is 4–20 times higher than that of the general population [1, 2]. This excess mortality is predominantly ascribed to cardiovascular diseases and diabetic renal disease [3, 4]. However, the excess mortality has decreased by 30%–40% during the last 50 years [2]. Furthermore, it has been shown that development of persistent proteinuria (PP) is a marker of a very poor prognosis in IDDM patients [5–7]. Thus, two questions arise from these epidemiologic observations:

How does development of persistent proteinuria influence the mortality of IDDM patients?
- 2.
Does the decreasing excess mortality of IDDM patients reflect a decreasing incidence of proteinuria?
