Abstract

Continual refinement in the techniques of cataract surgery has improved the prognosis for useful postoperative vision while at the same time decreasing the risk of complication. However, despite these improvements, the vitreous remains a “sleeping giant” that at best should not be aroused and at least should be violated only with respect. Cataract surgery causes a complex array of mechanical and biochemical changes in the vitreous that may result in pathologic alterations affecting either the vitreoretinal interface or anterior segment structures.
