Normal Endometrium

The endometrium, which lines the uterine cavity, is one of the most dynamic tissues in the human body, characterized by cyclic processes of cell proliferation, differentiation, and death in response to sex steroids elaborated in the ovary. An understanding of the variations in the normal morphologic appearance of the endometrium provides an essential background for the evaluation of endometrial pathology. During the first half of the menstrual cycle, all the components of the endometrium, including glands, stroma, and blood vessels, proliferate under the influence of estrogens; during the latter half, these elements respond to progesterone by the production of glandular secretions, and stromal and vascular alterations necessary for implantation of a fertilized ovum. The factors in the hypothalamus and anterior lobe of the pituitary gland that control and regulate hormone production in the ovary are discussed in Chapter 10.
