Abstract

More than 25 years have passed since Sir Russell Brock[1] proposed the idea of functional obstruction to left ventricular outlet. Within another year from that date, Donald Teare[2] published the pathological finding encountered in eight hearts with severe but asymmetric hypertrophy affecting the interventricular septum and because of cellular disarray likened it to a “muscular hamartoma”. Spodick and Littman[3] emphasized the malignant nature of idiopathic hypertrophy when symptoms had appeared in a publication about the same time. The physiologic and clinical features of obstructive cardiomyopathy were detailed by Braunwald with the conclusion that there was a dynamic obstruction to left ventricular outlet during systole.
