Summary

The qualitative and quantitative aspects of brain damage following stenotic or occlusive cerebrovascular diseases can be many. The paper takes into consideration cases of socalled “completed” stroke. The hypothesis is advanced that there are completed stroke patients in whom the brain functional disability appears to be related, totally or in part, to a blood supply still sufficient to satisfy completely their metabolic demand. In these cases addition of more blood can restore the impaired metabolism and therefore function. Such an arousal of the “sleeping” cerebral neurones can be obtained surgically, by properly increasing the circulation in the affected brain region.

The recognition of this peculiar relation between brain functional impairment and cerebral hemodynamic insufficiency, as well as the success of brain revascularisation, are dependent on several factors: 1. knowledge of the anatomic and functional situation of the cerebral circulation; 2. knowledge of the anatomic and functional conditions of the affected brain region; 3. correct choice of the modalities of surgical brain revascularisation.

These factors are analysed and discussed. The results obtained by creating surgically a new collateral brain circulation are presented.
