The Radiology of Paget’s Disease

Abstract

Paget’s disease is characterized by active remodeling of bone; its radiologic characteristics follow the transformation of the gross and microscopic anatomy that occurs in both cortical and cancellous bone. Bone is ultimately remodeled into a coarse trabeculated pattern that when pieced together characterizes the mosaic architecture that distinguishes the condition1–3 (Figure 1.1). The disease progresses through three distinct phases of remodeling: the early osteolytic or “hot phase,” the intermediate or “mixed phase,” and ultimately the late sclerotic or “cooled-off phase.”
