Susceptibility Testing of Medically Important Fungi

Abstract

In vitro susceptibility testing can provide results to predict the antibiotic microorganism interactions which should correlate with in vivo efficacy. Generally, the results are expressed as the minimal inhibitory concentration (MIC), the lowest concentration of an agent which inhibits the growth of the microorganism, or as the minimal fungicidal concentration (MFC), the lowest concentration of an agent which kills a statistically significant number of organisms. These values are then correlated with attainable blood levels to determine whether the organism is susceptible or resistant. Finally, clinical correlation of efficacy and susceptibility or failure and resistance must be confirmed. Unfortunately, these principles have not been well established with fungal pathogens and antifungal agents. Interpretation of the in vitro susceptibility testing of antifungal agents suffers from the lack of standardized procedures. Variables, such as the type and size of the inoculum, the medium, temperature of incubation, length of incubation, and the definition end points must be standardized in order to compare susceptibility and efficacy results.
