Abstract

Protein-energy malnutrition (PEM) is one of the more prevalent conditions in the world. A recent estimate indicated that perhaps one quarter of the worlds population could be considered to be malnourished (World Food and Nutrition Study, 1977). The vast majority suffer from undernutrition; however, an important and probably increasing percentage are overly nourished and there is reason to suspect that if too little is not good, too much may not be any better. Nutritional status, then, spans a wide spectrum of states of inadequacy, adequacy, and excess of which a limited segment is conducive to good or optimal physiological functioning. There is, in a sense, a middle way, for excess in either direction may lead to physiological changes or frank disease. However, this middle way is not necessarily midway between starvation and bulemia, but may rather be somewhat to the left of our current standards for weight, height, or dietary intake. This chapter will examine the relationship between nutritional state and immune function in human PEM in the undernourished of both mild-moderate and severe degree.
