Abstract

Narcotic substitution therapy, of which methadone maintenance is the most prominent example, was first introduced by Dole and Nyswander (1965). Methadone very quickly came into wide use for the treatment of opioid dependence and it has continued to be one of the most commonly used treatments. Much additional investigative work has been done since methadone maintenance began and this work has been conducted in diverse areas. These include evaluation of treatment outcome; descriptive studies of the behavioral and psychiatric disorders seen in methadone-maintained addicts; evaluation of ancillary treatments that may improve outcome, particularly those treatments that affect the psychiatric disorders of methadone-maintained patients; development of instruments to measure the spectrum of problems seen in opioid addicts; studies of the value of matching specific types of patients to specific ancillary treatments; testing the efficacy of maintenance drugs other than methadone; reviewing the outcome of patients treated with high versus low methadone doses; studying various methods of methadone detoxification; and evaluating the effects of opioid dependence and methadone treatment on neonates.
