Abstract

Cardiopulmonary bypass (CPB) during open-heart surgery causes massive activation of blood, resulting in an inflammatory reaction. One of the consequences of this activation process is impairment of the per-operative hemostasis. Invariably “oozing” of the wound area is observed,which requires drainage of the thoracic cavity until the first or second postoperative day and most often blood substitution. Oozing is a feature of capillary bleeding, primarily due to an impaired platelet function. Therefore improvement of the CPB circuit has been mainly directed to reduction of platelet damage. These efforts have resulted in the construction of a membrane oxygenator so to replace the bubble oxygenator. Although the platelet aggregatory function could be preserved by using a membrane oxygenator, the postoperative bleeding tendency was still considerable.
