Tubulo-Interstitial Lesions in Renal Allograft

Abstract

A mild tubulointerstitial nephritis can be seen in immunologicallyquiescent renal allografts as a result of an ongoing immunological attack. More severe lesions are observed during acute cellular rejection, where CD8+ lymphocytes and macrophages predominate. The use of cyclosporin can also induce tubulo-interstitial nephritis. The drug may produce reversible vasoconstriction, tubular damage and arteriolopathy with irreversible interstitial fibrosis. Appropriate adjustements of CsA dosage are mandatory to prevent progressive graft dysfunction.
