Diagnosis of Chronic Pancreatic Disease

Abstract

The present chapter deals with the diagnostic modalities used to investigate chronic pancreatic disease and the methods employed to differentiate chronic pancreatitis from carcinoma of the pancreas. The diagnosis of other conditions that lead to pancreatic insufficiency without pancreatitis or cancer (such as cystic fibrosis, primary pancreatic atrophy of childhood, adult pancreatic lipomatosis, or isolated lipase-colipase deficiency, etc.) are not a subject of this review, except to the extent that these conditions may also lead to pancreatic insufficiency, and thus abnormal pancreatic function tests. Nor is it intended to provide a classic description of the clinical features of chronic pancreatitis and carcinoma, as such descriptions can be found in any of the standard texts of gastroenterology or surgery. However, the relationships of the clinical and pathological features to the abnormal function and other diagnostic modalities are important and, therefore, are schematically shown in Figures 1 and 2. Note that the common denominator of both diseases is replacement of functioning parenchyma and the duct system with nonfunctioning tissue: inflammatory tissue and fibrosis in chronic pancreatitis and tumor in cancer of the pancreas.
