Abstract

A review of relevant records is an essential part of the information-gathering process of the neuropsychological evaluation. In addition to the physician’s medical appraisal of the patient, a wealth of information about the patient’s physical, cognitive, emotional, social, self-care, and community functioning is available in the reports and notes of the health professionals who treat the patient. Knowledge and understanding of these records are essential for the neuropsychologist who is a member of a hospital treatment team. Records about the patient’s premorbid activities can assist in the estimation of premorbid functioning. If the patient has been injured, the circumstances of the accident and emergency care can assist in understanding the patient’s condition.
