Abstract

Psychiatric inpatient treatment programs for children and adolescents encounter aggressive behaviors from many of their patients, and it is incumbent upon the professional staff of these programs to have an understanding of the importance of aggressive behavior as a clinical phenomenon and to have a clear approach to the management of violence on the unit. Aggression among hospitalized children and adolescents is multifactorial in etiology. The clinician must weigh the historical and diagnostic antecedents to the child’s behavior, as well as the current clinical issues and the inpatient environment itself to unravel the reasons for the child’s violent and destructive impulses. Parallel and coexistent with these imperatives are the programmatic needs to maintain a safe, nurturing, and therapeutic environment for all patients, and thus, the need to effectively intervene with aggressive behavior whenever it occurs on the unit. This chapter addresses the factors, clinical and programmatic, relevant to aggressive behavior on child and adolescent units and emphasizes the importance of accurate diagnoses in the evaluation of aggression. It reviews the application of contingency management techniques coupled to child skills training to decrease violent behavior, and it also reviews the psychopharmacological approach to aggression in this age group.
