Abstract
We would admit Patient A to the intensive care unit (ICU). In addition to blood cultures, blood gas measurements, determination of electrolyte levels, and electrocardiography, we would request that echocardiograms be obtained as soon as possible. It is likely that this would be followed with some form of invasive cardiovascular monitoring. In our institution, this would involve placement of a thermodilution catheter, although an arterial catheter, which also produces thoracic blood volume and lung water results, would be of more use. If the patient was peripherally cold and shut down, we would probably commence dobutamine 200 mg in 100 mL of normal saline at about 20 mL/h before inserting the Swan-Ganz catheter.
