Summary

Reperfusion of the infarcted myocardium is associated with an intense inflammatory reaction, which may extend ischemic injury, but is also crucial for cardiac repair, improving patient survival. Myocardial necrosis induces complement activation and free radical generation, triggering a cytokine cascade initiated by Tumor Necrosis Factor (TNF)-α release. In addition, chemokine expression is markedly induced in the infarcted myocardium and may have a role in regulating leukocyte infiltration, angiogenesis and granulation tissue formation. Bone marrow-derived stem cells may also be recruited in the infarct thus participating in cardiac repair. Elucidating the complex interactions and regulatory mechanisms responsible for infarct healing may allow us to design effective inflammation-related interventions for the treatment of myocardial infarction.
