Abstract

There are many potential coagulation concerns observed in women contemplating pregnancy, as well as in women who are pregnant, or in labor and delivery, and in the postpartum state. This chapter explores issues related to both bleeding and thrombotic disorders of young women, and how they impact the pregnant state. Also, complications of pregnancy that can cause changes in platelet count or changes in coagulation status are explored. These include the hypertensive disorders of late pregnancy, such as preeclampsia, eclampsia, acute fatty liver of pregnancy, the HELLP syndrome, and the devastating catastrophic complication of amniotic fluid embolism. Also, management of anticoagulation during pregnancy is discussed, especially in the light of the teratogenic potential of coumadin. Those hematologic conditions that can contribute to multiple miscarriages, such as the lupus anticoagulant, are also discussed.
