Abstract

In 1969, N. G. Kock published preliminary results of an intra-abdominal reservoir as an alternative to the conventional ileostomy. For patients needing proctocolectomy for ulcerative colitis or familial polyposis, the continent ileostomy still serves as a viable alternative to both the conventional Brooke ileostomy and ileoanal pouch. This chapter describes physiological principles of the pouch and evolution to the present continent ileostomy. Special attention is given to the surgical technique and the management of complications, including salvage procedures. Pouchitis and epithelial dysplasia of the pouch are discussed as well as the potential risk of polyps developing in patients with familial adenomatous polyposis and the surveillance of these patients. The indications for a continent ileostomy, and in particular the conversion of the failed pelvic pouch to continent ileostomy, are presented.
