Abstract
The total artificial heart (TAH) is a temporary biventricular support device designed to bridge patients with severe heart failure to transplantation. Patients with significant myocardial damage such as those with postinfarction ventricular septal defect, with intractable arrhythmias, and with mechanical valves or native valvular diseases who are not candidates for ventricular assist devices (VADs) are candidates for TAH implantation. The CardioWest TAH can also provide a rescue for patients with advanced cardiogenic shock, such as following massive myocardial infarction or failing on left ventricular assist device (LVAD).
The CardioWest TAH-t is implanted orthotopically as a bridge-to-transplantation. Currently the patient must remain in the hospital because they are connected to a cumbersome, albeit mobile, console that is soon to be replaced by smaller versions that will allow hospital discharge. While awaiting a transplant, patients have the time to recover and improve their health status. Patients improve their nutritional status and recover from any multisystem organ failures they may have suffered prior to transplantation. The CardioWest TAH-t is an excellent temporary option for patients who have failed medical therapy and are not candidates for an LVAD.
